We will provide your practice with several support tools to support your consideration of our services including Patient Education Brochures that are personalized for your office, Certificate of Medical Necessity/Rx forms to simplify completion and BMI/Epworth Sleep Score Reference Cards for your examination rooms. Many insurers specify criteria to establish a High probability of OSA to include an elevated BMI (>30) and high ESS (>10), thus we provide the reference cards with erasable markers that permit the patient to self-assess these metrics prior to your consultation.

We are pleased to provide a poster to increase awareness of your practice capabilities within your waiting room or exam room. Available in 18" x 24" Wall Mount or Table Top versions, the poster presents background information on Snoring and OSA and provides a convenient place to hold the Patient Education Brochures (available separately). A QR code is presented to enable patients with Smart Phones to scan immediate access to the SomnoGuard AP section of our website to obtain more comprehensive information.
