At Trillium Health, we use a patient/client-centered approach. This means that patients/clients decide what they want to work on and we provide them with the necessary supports to be successful. When we first meet with patients/clients, we help them to identify their needs and develop a plan to meet those needs. We advocate with other service providers to ensure that services are provided in a timely and respectful manner. We help the patient/client to gather all of the available information and then support the patients/client to make their own decisions.

So if you wish to have a say and be a partner in the care and services you receive at Trillium Health, consider becoming a member of the Consumer Council. We provide a safe and comfortable environment where you can learn first-hand what is happening at the agency and are encouraged to share your thoughts and ideas. The Consumer Council is open to all Trillium Health patients, clients, and their caregivers and meets the third Tuesday of each month in the Bud Shaw, Jr. Room at Trillium Health’s Monroe Avenue offices.

At Trillium Health’s main offices, located at 259 Monroe Avenue in Rochester, we provide a full range of on-site primary and specialty medical and supportive services to help you take charge of your health. From treatment adherence to smoking cessation to support groups to nutrition counseling and a food cupboard, everything you need is available here in one convenient location. In addition, satellite locations in Bath, Geneva, and on Central Avenue in Rochester offer a variety of supportive and referral services.

We provide the patient portal website for the exclusive use of established patients. The portal is designed to enhance patient-provider communications and improve the quality of care. We encourage all patients to take advantage of this convenience!
