Action Home Health is dedicated to providing skilled, professional care to its’ patients. As appropriate, agency professional staff visits the patient’s home to evaluate short-term and long-term health care needs. They work with the patient, their family, caregivers and the physician to develop a plan of care to meet each patient’s individual requirements. Referrals for additional services, equipment and supplies not covered directly by the agency may be referred to other providers.

The RN may also be evaluating the skills of the personnel providing the services including supervision of home health aides and other nurses, and assessing patient needs after hospitalization as applicable.

Home Health Care requires the prescription of a Doctor. We are committed to providing quality health care services to our patients while assisting them in achieving independent, productive living in the comfort of their homes.
