WE ARE HAPPY TO OFFER FINANCING THROUGH CARE CREDIT! (see details below) We provide each patient with a treatment plan, which includes a breakdown of all the applicable fees. It is our goal to inform our patients of all costs prior to treatment. Our office requires payment in full at the time of service. For those with Dental Insurance, co-payments and deductibles are also required at the time of service. For our patients with dental insurance, we will be happy to complete the necessary forms for their claim(s) as a courtesy. After we have filed the insurance claim, if benefits have not been received within 60-days, the entire or remaining balance becomes the patient's responsibility. (A refund will be given, should the insurance benefits come in later than the 60 days time limit). Following our original submission, any disputes of coverage, must be made by the patient (the customer) to the insurer (the provider). This is due to the fact that the insurance relationship exists solely as a contract between the patient and the insurance company - we are considered to be a third party providing the service. All remaining balances that remain after submission of a claim, are the financial responsibility of the patient.
