Benefits5 is a full-service health insurance agency that is fully licensed in the State of Michigan (License #:0085188). We specialize in employee benefit packages (employee medical, dental, vision, life, disability) as well as coverage for individuals & families. Our founder, James D. Bokshan, has been in the insurance industry since 1993 and carries several insurance degrees.

We are committed to keeping your personal information private. In the normal course of business, we will ask you to provide us with personal information about you or your employees in order to serve your insurance needs. We use your personal information to help you find and, if you choose to do so, apply for health insurance and/or other products that may be of interest to you, your family or your employees. We may also use your personal information to help the insurance carrier you choose process your application and to service your policy once it is in force.

The Michigan Psychological Association (MPA) has existed since 1935 and, in 1956, was incorporated as a 501(C)(6) non-profit organization. MPA is an organization of more than 1,000 psychologists working to advance psychology as a science and a profession and to promote the public welfare by encouraging the highest professional standards, offering public education and providing public service, and by participating in the public policy process on behalf of the profession and health care consumers. It is the Michigan affiliate of the American Psychological Association.
