Prostate Cancer Awareness The “Let’s Talk About It” Prostate Cancer Education Program is designed for African American men over 40 to help educate men about prostate cancer and their risk, and to encourage them to talk with their doctor about the disease. REACH US Community-based diabetes education, support groups and self-management classes for African Americans Sound Heart Community-based cardiovascular health promotion in diverse communities Tobacco Prevention and Control Grassroots advocacy/organizing and community-based initiatives in diverse communities United for Healthy Communities Street/community outreach and group-level interventions targeting people of African descent Breast and Cervical Health Program Community-based outreach, education and screening referrals for African American women Infant Mortality Prevention Community-based outreach and education in the African American community Community Health Services Program Interpretation, community education and health advocacy for individuals with limited English proficiency Refugee Health Advocacy Project Case management and English language training for immigrant and refugee families
