The discharge planning begins when you are admitted to a Skilled Rehabilitation Center after an acute hospitalization. During this process it is important to educate the patient and family regarding the patient’s needs (equipment and medication). A Home Health Company is often consulted to provide continued monitoring when discharged home. Follow-up with your primary care physician and specialist are encouraged.

Our physicians are board certified in their individual specialty including Pediatrics, Family, and Geriatric medicine, offering our services for scheduled, walk-in, and urgent care to people of every age.
