In 1998, Channel Marker, Inc. began providing community based rehabilitation services for youth (family, school and community based support). Psychiatric rehabilitation services for youth are currently offered in Dorchester and Caroline counties. We have also expanded our services to include the Primary Project in Talbot County schools. This program is funded by grants and donations.

Health Home is a healthcare delivery approach that focuses on the whole person and provides integrated healthcare coordination that includes primary care and behavioral healthcare. Channel Marker’s Health Home team of professionals includes a Health Educator, a Registered Nurse and a consulting Nurse Practitioner that work together, alongside the Psychiatric Rehabilitation team, to provide comprehensive assessment and coordinated care for adults with a serious mental illness.

It is the goal of the Health Home team to provide services that will help reduce emergency room visits and hospital admissions. Participants will learn to manage their care and be proactive instead of reactive. Staff will ensure timely and proper follow up care. Transitional care will vary by age and may include transitions to or from other types of care.

Your/your child's medical record (your protected health information) is kept in a secure location and only those employees who need access to that medical record for treatment, payment or health care operations, have access to your/your child's medical record unless you sign an authorization. It is our policy to reasonably limit disclosures of, and requests for, protected health information for payment and health care operations to the minimum necessary.
