Description

In 1957, Community Reach Center (formerly known as Adams Community Mental Health Center) was founded in Commerce City, Colorado. The clinic was staffed by a small visiting team of mental health providers from the University of Colorado Medical Center. In1967, Congress passed the Community Mental Health Centers Act, fulfilling President John F. Kennedy's vision to adopt a "bold new approach" to the treatment of mental illness in the United States.

A. Privacy Laws. Community Reach Center Systems is required by State and Federal law to maintain the privacy of protected health information. In addition, Community Reach Center Systems is required by law to provide clients with notice of its legal duties and privacy practices with respect to protected health information. That is the purpose of this Notice.

Community Reach Center's staff consists of a wide array of mental health professionals, including adult and child psychiatrists and psychologists, licensed professional therapists, licensed certified addictions counselors, early childhood specialists, nurses, case managers, special education teachers, and much more. Your team will work closely with you to identify treatment goals tailored to meet your specific needs, and, if necessary, connect you with other community resources that may enrich your treatment program.

Payment refers to the activities undertaken by a health care provider [including a mental health provider] to obtain or provide reimbursement for the provision of health care. For example, Community Reach Center Systems will use your information to develop accounts receivable information, bill you, and with your consent, provide information to your insurance company for services provided.
