Four Corners Community Behavioral Health, Inc. is licensed under Utah Code Annotated Title 62A to provide mental health and substance abuse services. It is the designated Medicaid service provider and sole source mental health and substance abuse program provider for Carbon, Emery and Grand Counties.

, LSAC received her LSAC license in 2003. She has worked for Four Corners since 1990 in several Programs during the 16 years with Four Corners. She has worked with people with Co-Occurring Disorders of mental illness and substance Dual Diagnosis, McKinney Homeless Program and Case Management on the Psycho-Social Team. She currently works in Adult Substance Abuse Treatment IOP, individual counseling, Relapse Prevention and Special Needs Group and Aftercare for those with Co-Occurring Disorders.

, LCSW, received a Masters Degree in social work from the University of Utah and a Bachelors Degree in Early Childhood and Elementary Education from the University of Oklahoma. Ms. Prazza joined the Four Corners team in 2006 and provides individual, family, and group therapy for adults and adolescents. Ms. Prazza worked previously in Salt Lake City for 20 years as a social worker, in outpatient, inpatient, and private practice settings.
