Our contracts are provider friendly to help ensure that your practice can continue to provide the services that you feel are best for your patients.

First Choice of the Midwest, Inc. can provide you with Geo reports, disruption reports, directories, and average savings information. Our team is dedicated to working with you and your staff to supply the information necessary to your business operations. First Choice of the Midwest, Inc. is a Preferred Provider Organization (PPO) that works with over 66 insurance companies. Although we are not the patient’s insurance company, we are dedicated to working with you and your staff to ensure that your relationship with the insurance companies that are contracted to access our network is mutually beneficial and straightforward.

We are dedicated to working to ensure that our network is providing the practitioners and facilities that you are seeking medical care from. If you do not see the practitioner or facility that you are currently seeing, or have been referred to, please feel free to complete the below form and we will reach out to this practitioner or facility to visit with them regarding joining our network. Please note that we will make every effort to add the practitioner or facility to our network, however, please note that they must meet the demographic needs as well as network and credentialing standards.

Founded in 1997 First Choice of the Midwest is an independently held Preferred Provider Organization corporately located in Sioux Falls, SD, with a second office located in Dubois, WY. We contract with employers, insurance companies and third party administrators seeking access to our comprehensive, high quality, cost effective network to support their health benefit plans.
