Information that may be collected includes, but is not limited to, an individual's name, address, telephone number, social security number, motor vehicle reports, policy number, premium and/or premium payment history, and credit reports. We also may collect from a claimant the claimant's name, address, telephone number, social security number, claim number, date of loss, type of loss, cause of loss, and the value of claim. We may also collect nonpublic personal information when you use our corporate web sites.
