The RN & Case Manager for the Connect Project provides medically-related case management services to Connect Project clients. The Connect Project was developed for the purpose of helping individuals living with HIV/AIDs who have a co-occurring mental health and/or substance abuse disorder maintain housing placements. Within the scope of the RN licensure, the RN will act as a conduit to client’s medical provider, provide nursing support, and collaborate with the multidisciplinary team in order to improve overall function and produce better placement outcomes.

The Case Manager for the Connect Project has direct clinical care responsibilities for vulnerable individuals, providing a broad array of case management and supportive counseling. The Connect Project helps individuals living with HIV/AIDs and who have a co-occurring mental health and/or substance abuse disorder maintain housing placements. This role provides services to Connect Project clients that improve their overall function and result in better placement outcomes. A master's degree from an approved school of social work and a current Maryland license as a LGSW, LCSW or LCSW-C is required.

Louise Treherne has oversight of Behavioral Health Services, Supportive Housing Initiatives, Public Grants, Reporting, and Program Development, as well as our purchase of service programs in surrounding counties. She is a health care administrator with over 30 years of experience in HIV/AIDS, mental health, addiction, case management, and homeless health care service delivery. Louise is a member of the National Health Care for the Homeless Council Clinician's Network and Respite Care Steering Committee.

The Human Resources Generalist contributes to the practices and projects that will provide an employee-oriented, empowered culture at HCH. This role collaborativesly manages a broad spectrum of HR functions, working with the HR team, staff, managers and outside partners. The HR Generalist must role model a proactive, thoughtful and balanced approach to conflict and ambiguity. A bacherlor's degree is required as well as 2-3 years experience in human resources, preferably in the health care setting. Must-haves include experience in recruiting, HRIS, compensation and benefits.

2000 : Health Care for the Homeless becomes the first independent HCH project in the country to receive accreditation from the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). The Board of Directors adopts a new mission statement: "We provide health-related services, education and advocacy to reduce the incidence and burdens of homelessness." The accompanying strategic plan reaffirms the agency’s commitment to advocacy and expands services to certain populations at risk of homelessness.
