Front Row, Left to Right Marianne Gaudio Having worked 25 years as the office manager of a Long Term Care Facility, Marianne entered the field of hospice. After just a few years, she and five friends embarked on their journey into a hospice of their own. Nora Giurici BSN, RN, CHPCA Nora worked in hospital finance and administration prior to becoming a registered nurse in 1992. After several years exploring various areas of nursing she found her "home" in hospice. After managing a large, multi-state hospice for 10 years, she, together with her business partners, founded Homeside Hospice. Back Row, Left to Right Theresa Scholl RN Theresa spent 24 years in geriatric long term care, where she was part of the management team responsible for MDS coordination, Education, and Infection Control before becoming Assistant Director of Nursing and Director of Nursing. In 2000, Theresa joined a large hospice as Director of Clinical Operations. In 2005, Theresa and five of her respected colleagues founded a grass roots hospice. Homeside Hospice is dedicated to excellence in end of life care and focuses on patient dignity, comfort and inner peace.
Each patient and family is assigned an interdisciplinary team of professionals and volunteers. Under the direction of the patient's physician and our hospice medical director, the team works together to provide each patient and family with unique, personalized care. Each team member brings skills and expertise to the patient to provide the highest quality of comfort care possible. The entire range of physical, emotional, and spiritual care are factored into a patient's individualized care plan. This plan is regularly reviewed and adapted to meet the evolving needs of the patients and their families.
Utilizing hospice in an assisted living facility provides an additional level of care and support both to the resident and to the facility. It can provide the resident with the additional level of support needed to remain at the facility rather than being transferred to an alternative care setting.
Right of access to inspect and copy your health information : You have the right to inspect and copy your health information, including billing records. A request to inspect and copy records containing your health information may be made to Barbara Sost, LPN, Director of QAPI and Compliance at Homeside Hospice, 732-381-3444. If you request a copy of your PHI, we may charge a reasonable fee for copying and assembling costs associated with your request. You have the right to request that we provide you, an entity or a designated individual with an electronic copy of your electronic health record containing your health information, if we use or maintain electronic health records containing patient health information. We may require you to pay the labor costs incurred in responding to your request.
For Payments : Homeside Hospice may use and disclose your PHI so that we or others may bill and receive payment for the care you receive from us. For example, we may be required by your health insurer to provide information regarding your health care status, your need for care and the care that Homeside Hospice intends to provide to you so that the insurer will reimburse you or the hospice for services provided and received.
