We're happy to provide you with all the necessary new patient forms prior to your first appointment. Simply click on the "New Patient Information" icon in the margin. From there, you can quickly print the forms you need. If you don't have Adobe Acrobat Reader, you'll find that we provide a free download version for your convenience.

As a courtesy, we will be happy to assist you in filling your dental insurance. We are an out of network provider for all insurance companies except Delta Dental . We collect your estimated portion on the day of service and will wait 30 days to obtain the balance from your insurance. The patient or guarantor is responsible for the entire fee. Any portion not paid by your insurance company in 30 days will be collected from the patient. Dr. Stephens and Dr. Gatewood will provide dental care based on your dental needs and will not be based on your insurance coverage.

If necessary, we also offer complimentary nitrous oxide or a prescription oral sedative prior to treatments for patients who are extremely anxious or where the procedure is very complex. We're also equipped with an anesthesiologist to provide services for certain procedures. When you come in for your evaluation, just let us know how you're feeling, and we'll work together with you to put you completely at ease.

CEREC stands for Chairside Economical Restoration in Esthetic Ceramics. The high-tech computer and milling machine manufactures custom crowns, inlays, onlays and veneers onsite. To you, this means one visit restorations instead of attending two dental visits and wearing a potentially faulty temporary.
