Description

We believe that homelessness is not a choice, and that given the opportunity for safe, affordable housing, everyone prefers a permanent place to live. Our Homeless Project serves individuals in Duluth who are homeless and have a mental illness. Many of those we serve also have health problems and substance abuse issues. We provide community outreach to locate homeless persons, and assist them to obtain and keep permanent housing. We also provide case management, employment assistance, transportation, benefits assistance, and crisis intervention. We work closely with other service providers, including St. Louis County Public Health and Human Services, Duluth Police Department, CHUM, The Union Gospel Mission, Damiano Center, Salvation Army, Center City Housing, Life House, AICHO, Center for Alcohol and Drug Treatment, and Northeast Regional Correction Center. The Homeless Project is located at 315 N. 2nd Avenue West, Duluth, MN.

DUTIES: Direct patient care. Diagnose and treat psychiatric patients with chronic and acute mental health problems and associated general medical and addiction co morbidities. Improve the mental health of our communities by providing clinical and administrative services, education, research, and advocacy. We work in a team-based model which is driven by community need. We are dedicated to bringing high quality, accessible services to people where they are, and to supporting and nurturing health clinicians.

We provide consultations and ongoing services for older adults with depression, anxiety, and other mental health problems associated with aging and physical health problems. Initial evaluations are typically done by a Geriatric Nurse Practitioner with psychiatric expertise. MINIMUM QUALIFICATIONS: MINIMUM QUALIFICATIONS: Master’s degree in nursing with credentialing from the ANCC (Family or Adult Psychiatric Mental Health Nurse Practitioner or Clinical Nurse Specialist). DESIRED QUALIFICATIONS: DESIRED QUALIFICATIONS: DNP degree a plus. Ability to treat children and adults is desired, but not required. Previous work experience in a community mental health clinic or other psychiatric clinic setting and in primary care settings as a NP/CNS. Also, knowledge of addictions and experience working with adults as well as other age populations; knowledge of community psychiatric systems and resources. Minimum of five (5) year of experience as a psychiatric nurse.

Eating disorders are highly complex, bio-psychosocial problems which require specialized care for effective treatment. The Human Development Center’s Eating Disorders Treatment Program, led by Dr. Carolyn Phelps, provides expert specialized care to people of all ages suffering from Anorexia Nervosa, Bulimia Nervosa, and Binge Eating/Compulsive Overeating, regardless of gender.
