Serving the greater Twin Cities area offering a variety of healthcare services since 2002! We provide Home Care, Foot Care and specimen acquisition and are proud of the unique, patient-centered, responsive care we provide to our patients.

Our goal is to provide all levels of home care services in collaboration with physicians and other members of the health care team - allowing clients to remain in their own homes. Our nursing staff welcomes the opportunity to assess the needs of the home bound individuals. This will help determine if the services we offer will enhance the goals and wishes of the client while supporting a healthy, stable and safe environment. It is an honor to be invited into the homes of our clients and we strive always to exceed the hopes and expectations of those we serve. We provide both professional nursing services and supervised Home Health Aide and Physical Therapy services. We are available for routine calls from 9am to 5pm daily and for emergencies 24 hours daily. An RN supervisor is available to respond by phone to after hour emergencies.

Hospice Advantage is a Medicare, Medicaid Certified program providing superior end-of-life care services. Hospice Advantage was started to better serve the needs of terminally ill patients in the US. In November, 2004, Hospice Advantage was Founded by Rod Hildebrant, MSA, CHCE, a health care executive with more than 25 years experience in the home health care and hospice industries.

In-Home Lab Connection and Home Lab Partners have filed with the federal government to establish a foundation which will be called Home Health Connection. Once the foundation is established, grant proposals will be written in order to fund the services that we provide to those who are unable to cover the costs.
