Description

Dr. Garner was highly involved in various student organizations, sports, and leadership roles while attending Halifax County High School. She continued her involvement and obtained a Bachelor of Science Degree from Fayetteville State University. The final destination was the University of North Carolina- at Chapel Hill School of Dentistry where she obtained a DDS degree. One of the greatest lessons learned while in dental school, aside from the academic materials taught on a daily basis, was service to others. Upon graduating from dental school, Dr. Garner has served as a dental associate for over 3 years.

Jennifer Morris-Mrs. Jennifer Morris-Mrs. Morris grew up in Greensboro, North Carolina and is a Registered Dental Hygienist. She graduated from Guilford Technical Community College with her Associates of Applied Science in Dental Hygiene in 2006. Jennifer brings over five years of dental experience in cosmetic and general dentistry. She loves interacting with patients and always strives for detailed explanations of all dental procedures and oral hygiene instructions.

Access: You have the right to look at or get copies of your health information, with invited exceptions. You may request that we provide copies in a format other than photocopies. We will use the format you request unless we cannot practicably do so. You must make a request in writing to obtain access to your health information listed at the end of this Notice. We will charge you a reasonable cost-based fee for expenses such as copies and staff time. You may also request access by sending us a letter to the address at the end of this Notice. If you request copies, we will charge you $0.10 for each page, $10 per hour for staff time to locate and copy your health information, and postage if you want the copies mailed to you. If you request an alternative copy, we will charge a cost-based fee for providing your health information in that format. If you prefer, we will prepare a summary or an explanation of your health information for a fee.
