Jewish Family Service’s Senior Service Program is seeking a half- time professional to provide care management for seniors and adults with disabilities living independently in the community, and to help grow our care management program. Qualifications: MSW or RN with current MN license or relevant experience, knowledge of community resources, experience working with older adults and people with disabilities, experience in care management, excellent verbal and written communication skills, and an ability to work well in a multi-disciplinary team. Familiarity with Jewish religion and culture is a plus.

Jewish Family Service will continue to be acknowledged as a significant, culturally sensitive community resource with programs that identify and meet the changing needs of clientele in the greater St. Paul area. We provide comprehensive counseling services for individuals, couples, children, and families. Our licensed and experienced staff offer confidential help with concerns about depression, anxiety, relationships, parenting, childhood behavior, loss, and aging.
