We recognize that your time is valuable so we have included our secure Online Patient History Form this eliminates the need to fill out papers when you come in. If you prefer to print out your paper work, simply download the Printed Version of the Patient History Form. Print the form, fill it out and fax or mail it back to us.

At Korver Eyecare Center, we provide information on advanced lenses technology, so that eye wear can be custom-designed.

Anita joined the Korver Eye Care team in 1997. She has over 20 years of experience in the optical field. She is a member of the National Academy of Opticianry. Director of the Vacation Bible School at her church. Anita has taught Sunday School for many years. She is also a Post Prom Sponsor and the active mother of 2 (son and daughter).
