Health Information Exchange. We participate in one or more electronic health information exchanges which permits us to exchange medical information about you with other participating providers (for example, doctors and hospitals) and health plans and their business associates. For example, we may permit a health plan that insures you to access our records about you to verify a claim for payment for services we provide to you. Or, we may permit a physician providing care to you to access our records in order to have up to date information with which to treat you.
Our staff is dedicated to providing care to all patients both during their hospital stay and through discharge planning to make their transition home easier for both patients and families. Assistance is available with discharge planning, insurance questions, advance directives as well as coordinating other services.
