Mail Cache

Description

Dear Mail Cache Customer: By United States Postal Service regulation, you and any other adult using your mailbox to receive mail, MUST sign a postal form 1583 in duplicate. This can easily be done with the aid of any Mail Cache employee at our front counter. If you are not in Anchorage, you will need to fill out the two copies of this postal form (available for download at the bottom of this page), then have your signature witnessed and stamped either by a local Notary Public or any Postmaster at your local U.S. Post Office. Should there be another person using the mailbox for receipt of mail, they will have to fill out and sign a separate 1583, in duplicate, as well, unless they are your spouse, in which case a single 1583 can be used for both of your names and signatures. If you have any minor children receiving mail at your Mail Cache mailbox, all that is required is their names and current ages, listed in section 13 of your postal form 1583. For your convenience, here are step by step instructions to aid you in filling out the PS form 1583: Please leave sections 2, 3, 4 and 6 BLANK. We, as your agents, will fill them in. 1. Start by dating the form with the date you will be signing it, in section 1 in the upper left top corner of the form. 2. Enter your name (and your spouse's name, if applicable) in section 7. 3. If you have any minor children who will be receiving mail at your Mail Cache mailbox, list their names and current ages in section 13. 4. In section 8 fill in your home address and home telephone number. If you have no home telephone number, write the word "none" where you would normally write your telephone number. 5. If you are NOT using your Mail Cache mailbox for the purpose of receiving mail for a company, please mark the "No" box in section 5 and skip the next instruction and go directly to instruction line number 7. 6. If you ARE using your Mail Cache mailbox for the purpose of receiving mail for a company or corporation, please mark the "Yes" box in section 5. After you check the "Yes" box, please list the names of any other members of the company or corporation that would be receiving mail at your mailbox in section 13 (note: each of these people will need to fill out and sign their own postal forms 1583). Now please list all company name(s) in section 10, and then list their business address and business telephone number in section 11. If you do not have a business address, and operate the business out of your home, use your home address in this section. If you have no specific business telephone number, and use your home telephone number for your business, list that telephone number in the space provided in section 11. List the kind of business you run in section 12. If your business is a corporation, list the names and addresses of its officers in section 14. Now list the name of the county and state the business(es) are registered in, as well as the date it was registered in that county and state in section 15 of the form. 7. You (and your spouse, if applicable) must list two types of ID and their corresponding ID numbers, on the lines provided in section 9.
