Changes on the form are necessary to accommodate the 10-digit National Provider Identifier, and for consistency between paper and electronic claim formats. As of May 23, Medicare required institutional paper claims, including adjustment claims, on the UB-04. However, during a transitional period, Midlands Choice is continuing to accept claims on the UB-92.
