USES AND DISCLOSURES OF HEALTH INFORMATION We may use and disclose health information about you for treatment, payment, and healthcare operations. For example: Treatment: We may use or disclose your health information to a physician or other healthcare provider providing treatment to you. For example, we may send a report and/or copy of your films to the physician who orders a study. Payment: We may use and disclose your health information to obtain payment for services we provide to you. For example, your health plan may require us to obtain prior approval so that your plan will pay for your study. Healthcare Operations: We may use and disclose your health information in order to run the office and make sure that you and our other patients receive quality care. For example, we may use health information about our patients to help us decide whether certain new treatments are effective. We may disclose health information to your health plan to help them provide or improve care, reduce cost, coordinate and manage health care and services, train staff and comply with the law. Required by Law: We may use or disclose your health information when we are required to do so by law.
