Reaching Out During the flooding of 2010 Primary Health Care staff assisted 275 families at the Grandview Trailer Park. They helped with the medical needs, as well as transported the families to safety and shelter. It was a collaborative effort between PHC and our partners within the Polk County Housing Continuum. PHC family support workers continue to assist with these families and others in their recovery efforts, with additional support from the Red Cross and FEMA for longer term solutions. Who We Are We are a nonprofit community health center dedicated to serving the medically insured, uninsured and underinsured with their health care needs. We strive to find and successfully treat medical conditions before they become serious enough to require hospitalization or emergency treatment.
