These plan designs include High Deductible, Health Reimbursement Arrangements using PPO, POS and HMO networks. Group Dental Insurance Plans Available for groups of 2 – 5,000 offered from top-rated dental providers which provide the most competitive rates and best values in dental coverage. Allstate Voluntary Benefits: For groups of 10+: We offer benefit plans that express underwriting with only 3 questions asked in order to purchase Life, Disability, Critical Care, Accident and Cancer coverage for just a few dollars per paycheck. Conditional Guarantee on Life and Disability, Accident and Cancer coverage is available for larger groups. All voluntary benefits are completely portable, with guaranteed level rates after leaving employment College Awards Plan Receive up to $1,000 in College Tuition points (each point = $1 in tuition) plus a 5% annual match on 401(k) balance. Example $50,000 in 401-K assets X 5% = $2,500 in tuition points. And each year you receive another 5% match on the year end 401-K balance. With no maximum limit on what you can earn. These points can be given to your children, niece, nephew, grandchild or anyone who is in 10th grade or younger. VSP Vision Plan Provides for annual ophthalmologist eye exam, lenses or contacts annually and frames every 2 years. The plan can be set up on a voluntary basis or company paid. 401(k) Retirement Savings Plan Available from dozens of the leading Retirement Services, such as Principle Life, John Hancock, ING, and Fortis. Many of these Retirement Plans provide for the elimination of fiduciary liability to Owners/Trustees with $0 start up fees.
