In 2005, this 75 year old woman presented with vision of 20/50 in the right eye and counting fingers in the left eye (Figure 1).The subfoveal classic choroidal neovascularization in the right eye (Figure 2). The patient began treatment with Avastin in December of 2005. Treatments were continued about every 8 weeks for 4 years. As of Januray 2010, the patient was stable at 20/60 in the right eye, as seen on the OCT (figure 3) She was then switched to a 10 week Avastin interval, and her vision dropped to 20/70. An OCT (figure 4) taken in March of 2010 shows residual/recurrent intraretinal thickening. This case shows a patient that is still dependent on regular anti-VGEF injections. A seemingly minor change in schedule (8 to 10 weeks) caused a recurrence of the subfoveal neovascularization.
