These forms explore your current complaint(s), past and present medical conditions as well as family medical history. To save time these forms can be downloaded and filled out prior to your first visit. Please bring your insurance card so we can verify your insurance benefits, or arrange other payment methods. The Chiropractic Exam Once you're finished with your paper work Dr. Sheradin will sit down with you to review the information you provided, and discuss your medical history and complaints.
