These contain advice to travelers, some answers to questions asked by fearful flyers, and commentary on (at the time) current aviation events. I will post a new article each week. Who Do You Trust? (Choose Your Pilot) Pilot Error 　 Suppose your pilot was to make an honest mistake, would your aircraft crash? An old saying in aviation says that there has never been a perfect flight. That saying suggests that many mistakes are made on each flight. Robust is a word that describes modern cockpits. That means if someone makes a mistake, that it will be caught and fixed, with no harm done. The concept of robustness extends beyond the cockpit. Professor James Reason, of the University of Manchester, England invented an accident prevention program which has been universally embraced in aviation. His model, although highly mathematical, is symbolized by a bullet being fired through several slices of Swiss Cheese. The bullet symbolizes a threat. Each slice of cheese symbolizes part of the aviation safety system. One is for the design of the aircraft, one for construction; one for procedures and training, and one for piloting. If the bullet hits cheese, and not a hole, then an accident is prevented. But if the bullet happens to hit holes in every slice, then the threat produces an accident.
