AGENTS APPLYING ONLINE PLEASE FILL OUT THE THREE FORMS BELOW. THIS INCLUDES OUR AGENCY AGREEMENT, BACKGROUND INFORMATION AND BACKGROUND AUTHORIZATION. ALL INFORMATION WILL BE KEPT STRICTLY CONFIDENTIAL. WE JUST DO A STANDARD BACKGROUND CHECK. INDIVIDUALS WHO WOULD LIKE TO BECOME AN AGENT FOR US PLEASE READ OUR INSTRUCTIONS BELOW. Please print out The Background Information and The Background Authorization Form. Once you have done this fill them out and fax them back to us. Our fax number is 708 216 0206. It just takes a couple of days to process your application. After we have processed your application we will send you our completed Agency Agreement for you to also fill out, sign and send back.
