All co-pays, co-insurance and deductibles must be collected at the time of service. We are happy to provide the service of billing your insurance company on your behalf and will accept payment from them directly but any monies owed on your account are ultimately your responsibility per the financial agreement you signed. The clinic can not be responsible for disputing or appealing the way your insurance has processed payment on a claim. In the event we are not paid in full by your insurance company (less any contractual write-offs we are to take with insurances we are contracted with) you will be balanced billed any remaining charges after your insurance has addressed a claim and required to pay us directly and work out your difficulties with your insurance carrier.

Regular screening for these blood lipids is inexpensive, simple, and gives you a better picture of your overall heart and vascular health. In the event that you have unsafe levels of these blood components, steps can be taken to correct these levels with diet, exercise, and simple lifestyle modifications under a conservative treatment plan prior to necessitating more aggressive treatments such as statins. To provide this service to our patients, our clinic uses a local diagnostic lab called Quest Diagnostics. The blood samples are drawn in our office by Dr. Seth Alley to provide a comfortable, stress-free environment with flexible scheduling options. In some instances, your medical insurance may even cover the cost of these procedures. If you do not have laboratory coverage, we will work with you to keep the lab costs to a minimum so that you do not have to sacrifice your health during these trying economic times. If you are already seeking care from another doctor (the “attending physician”), you can get a referral from that doctor for chiropractic care or change your attending physician to your chiropractor. Patients can change their attending physician up to two times after the initial choice. In the state of Oregon, treating physicians must qualify with the state’s Workers’ Compensation Board to treat patients under this system. Dr. Seth Alley, DC, CKTP and the Sylvan Chiropractic Clinic and Wellness Center are fully qualified with the state’s offices to provide services to injured workers.
