Payment: Unity Hospice Care may use and disclose your health information so that we or others may bill and receive payment for the care you receive from us. For example, we may be required by your health insurer to provide information regarding your health care status, your need for care and the care that Unity Hospice Care intends to provide to you so that the insurer will reimburse you or the hospice for services provided and received.

• Right of access to inspect and copy your health information: You have the right to inspect and copy your health information, including billing records. A request to inspect and copy records containing your health information may be made to the Vice President of Clinical Operations in Germantown, TN at (901) 756-7322. If you request a copy of your health information, we may charge a reasonable fee for copying and assembling costs associated with your request.
