Community Based Residential Facility (CBRF) SKILLED NURSING AND REHABILITATION SERVICES Short Term Care – Rest Haven provides in-house physical therapy, occupational therapy and speech therapy fives days a week or as prescribed by a physician. Specialized programs are designed by professional therapy staff to achieve an individual ’s goals and allow the individual to remain as independent as possible. Assistance is provided by social service and rehabilitation staff members when treatment is complete to ensure short-term residents a safe and successful return home. As needed, home visits by therapy staff are provided in order to assess necessary home adaptations or potential hazards. Long Term Care – In some situations, long term care becomes necessary. The challenging needs of the long-term care resident is provided in a reassuring, home-like environment. Specially trained staff realize the importance of giving quality care in a kind, understanding manner while maintaining the resident ’s sense of dignity and respect. Confusion, forgetfulness and anxiety are frequently experienced by the elderly. Because of this Rest Haven ’s staff is familiar with such situations and are trained to offer support, reassurance and security in order to alleviate fears. In accommodating these special considerations, we provide a 24 hour electronic security alarm system, activity programs designed to stimulate and encourage mental activity and one-on-one contacts with social service staff.
