How your HEALTH INFORMATION may be used To Provide Treatment We will use your HEALTH INFORMATION within our office to provide you with medical care. This may include administrative and clinical office procedures designed to optimize scheduling and coordination of care between health care providers and business office staff. In addition, we may share your health information with other health care personnel providing you treatment.

- Certified Optometric Vision Therapy Technician . To be certified an individual must be employed by a Fellow (FCOVD), provide documentation of 2000 hours or 2 years of direct clinical experience in vision therapy; or 1000 hours of clinical experience if the individual holds an AA degree or higher with emphasis in the behavioral sciences. Submit written answers to a series of Open Book Questions dealing with various aspects of vision function, testing and therapy.

- a physician (doctor of medicine (M.D.) or doctor of osteopathy (D.O.) who specializes in the comprehensive care of the eyes and visual system in the prevention of eye disease and injury. The ophthalmologist has completed four or more years of college premedical education, four or more years of medical school, one year of internship, and three or more years of specialized medical and surgical training and experience in eye care.
