Our practice is required by law to maintain the privacy of your identifiable Protected Health Information and to provide individuals with notice of our legal duties and privacy practices with respect to such information.

We provide a comfortable and equipped work area for the visiting monitor. In addition, Kremer’s Market is next door, which offers quality foods and a daily lunch and dinner menu. The site is also located near a retail area with a variety of restaurants and retail establishments. Downtown Cincinnati and Northern Kentucky’s Covington and Newport riverfront entertainment areas are about a 10-15 minute drive away.

We may use and disclose Protected Health Information for your treatment and to provide you with treatment-related health care services. For example, we may disclose Protected Health Information to doctors, nurses, technicians, or other personnel, including people outside our office (for example, your pharmacy) who are involved in your medical care and need the information to provide you with medical care.
