Description
Master's degree in human service field with at least one year experience in mental health services preferred OR Bachelor's degree in Social Work, Psychology, Counseling or riminal Justice with three years mental health experience, one of which must be in Service Coordination. Knowledge and xperiencewith CANS/ANSA preferred. Knolwedge and understanding of the needs of the chronically homeless is preferred, as well as experience with, and passion for, assisting homeless individuals and families to gain permanent housing.
Once referred to Chartiers, the Service Coordination staff help the individual locate housing. The individual directly holds the lease to the property and Chartiers will use the HUD funding to pay a portion of the individuals rent. Operating as a Shelter Plus Care model, Chartiers staff will continue to provide services as needed and the individual is required to follow the terms of the lease. Chartiers HUD grant is part of the 8.3 million in federal funds received by Allegheny County to expand homeless programs.
