Assessment of client's capabilities and limitations in all areas of independent living. This includes tasks of daily personal care, homemaking, community activities, and social connections. Additionally, home environment safety is studied to minimize the risk of falls or accidents. Care Planning pulls together the information from the Assessment into a workable, step- by-step action plan. The plan, also, incorporates input from family members and other caregivers, the client, and relevant professionals. Service Coordination and Monitoring includes locating, screening, scheduling and monitoring all caregivers, contractors, and other service providers; coordinating medical and rehabilitation appointments, transportation, social programming, and follow-up services; and ongoing communication with concerned family members. Advocacy for patient's rights and services within the healthcare delivery system, entitlement programs, or within day or residential programs Consultation/Evaluation can be a one-time meeting with family members or other concerned parties on urgent issues, or meetings scheduled only on an as-needed basis. Caregiver Coaching provides family caregivers with ongoing professional assistance or guidance to solve difficult problems or adapt to the changing needs of their loved one.
