Services will be provided by our primary health care team which consists of a pediatrician, internal medicine physician, health educator, family practice physician assistant, EMT/medical assistant and support staff.

QCHC provides primary preventive, chronic and episodic care services to thousands of residents in Philadelphia. We provide care to many patients that do not have the ability to pay for quality health services. QCHC offers medical, dental, nutrition, behavioral health, case management and integrated services to all patients.

Operation Brown Sugar (OBS) is part of QCHC Chronic Care Model approach to outcome-based health care. The program started in 2003 with a small number of patients and now has grown and spread to include all our sites and all our eligible patients. OBS is for persons with Type I or Type II Diabetes; the program is designed to educate participants about healthy lifestyles for diabetics and to help them maintain their health. This program is customized for each individual, but also provides group support meetings.
